Outreach Services

Hume-Lee has two transplant outreach coordinators who serve as liaisons for our patients and community health care professionals. To facilitate ongoing education and communication, our outreach coordinators are available for the following services:

  • Provide transplant education to the public and talk with pre-end-stage renal disease patients, civic groups, and dialysis patients who would like to learn more about the transplant process.
  • Assist health care professionals with the patient referral process and ongoing communications once a patient has begun transplant evaluation.
  • Schedule continuing education for medical professionals interested in learning more about organ transplantation and donation, and the robust service line at the Hume-Lee Transplant Center.
