ACO REACH Helps Us Provide More Coordinated Care for Patients with Medicare

Our medical practice, called MCV Physicians, is taking steps to provide better quality care for patients with Medicare. By joining ACO REACH, an accountable care organization (ACO) that works directly with Medicare, we are now connected to other health care providers who share a common goal: to work together to offer more care coordination than Medicare typically provides, at no cost to our patients.

Through ACO REACH, patients with Medicare can get more access to certain services, such as care management and home visits after hospital stays—and may even qualify for cost-sharing help for chronic care management.

Take the First Step Today

You may have received a letter letting you know we joined ACO REACH. If you did, we’d love for you to fill out the form that came with it. This form will help us verify if your primary care provider is with MCV Physicians (also sometimes referred to as MCV Associated Physicians), which means that you’d qualify for the program.

Your primary care provider is the doctor you typically see for regular care and checkups, as well as for help with other health problems, like asthma, diabetes and hypertension.

Your Medicare Benefits Won’t Change

Filling out this form will help us determine if ACO REACH can help coordinate your care. But you don’t have to complete it. If you do return the form now, and then later decide you want to see another doctor, don’t worry: It’s okay to change your mind at any time.

Whether you choose to complete the form now or not, your Medicare benefits won’t change. And that means you can:

  • Continue to see any doctor or health care professional.
  • Receive the same Medicare benefits.
  • Visit any hospital that accepts Medicare at any time.
