Please call (804) 360-4669 to make an appointment for yourself or a family member – a physician referral is not required. However, check with your insurance plan on pre-authorization requirements.
New patients
Preparing for your first appointment
Your first appointment is with our neurologist. It is typically 90 minutes – including check-in and time with the doctor. The clinic coordinator will mail you a new patient packet with forms to be completed before your first appointment.
Haven’t received your new patient packet?

Prior to your appointment, request that your primary care or current neurologist send your medical records, including any radiology and imaging from the past year. If you have had genetic testing in the past year, please forward the lab reports.
For your first appointment, please bring:
- Your insurance cards.
- Completed new patient packet.
- Any additional medical records you have that have not yet been forwarded to our office.
- Any films or CDs (not simply the report) of radiology testing (MRI, CT, x-ray) relevant to your condition that you have had completed in the past year. These are particularly important and if necessary, you may have to retrieve these from the imaging testing location.
- The pill bottles for all the medicines you are currently taking. Our staff and neurologists may need to see the actual bottle(s) with the name(s) and dosage(s) listed.
Resources and support groups
We have many resources and educational opportunities for our patients, families and caregivers. You do not need to be a current patient to participate.
