Refer a Patient

Referrals from a primary care physician are required for most visits since we are specialists. The exceptions to this are difficulty swallowing, hoarseness and suspicious tissue masses. It is helpful for patients to bring copies of any pertinent medical records and tests with them on their first visit.

Referrals can be faxed to 804-807-7951 with a cover letter stating the date of their appointment. Patients should bring their actual X-ray films with them if possible.

• For patients with a cancer diagnosis, please fax records with a cover letter stating the date of appointment.
• For smell and taste patients, please fax records with a cover letter and a nurse will call the patient to set up an appointment.
