Temporary Closure of Release of Information Window, Care Building 2nd Floor

EFFECTIVE March 26, 2020

Due to the Corona (COVID 19) Virus, in order to keep our patients, visitors and staff safe, the Release of Information Window will be closed indefinitely.

 

ALTERNATIVE ACCESS TO PATIENT HEALTH INFORMATION:

Access My VCUHealth Patient Portal – Information normally available: Inpatient Discharge Summaries, Clinic Visit Notes, X-ray Reports, labs results. To sign up for My VCUHealth Patient Portal, visit https://www.vcuhealth.org/my-vcu-health and you can sign up if you don’t have an account or log into your patient portal.

You may also download an authorization form to request copies of your medical records and/or radiology images from the “My VCU Health Patient Portal” page by:

  • Selecting the “Patients and Visitors” tab at the top of the page
  • Selecting the “Medical Records” link. This will take you to the “Release of Medical Information” page.
  • Select the “Authorization Form” link to download the authorization form.

 

You may submit your completed authorization form via fax, email, or US Mail:

For upcoming patient medical appointments – Ask your provider’s office/treating facility to fax/e-mail over a request on their letterhead to obtain copies of your medical information.
