Consolidating outpatient internal medicine and neurology services

VCU Health CMH Logo

VCU Health is consolidating our outpatient internal medicine and neurology services in South Hill. Those services, currently offered at various locations in the community, will move to our state-of-the-art C.A.R.E. building adjacent to VCU Health Community Memorial Hospital.

As a result of the move, we are closing the following clinic locations:

  • The last day providers will be seeing patients at VCU Health Internal Medicine at South Hill (412 Durant Street, South Hill, VA 23970) will be June 26, 2020.

  • The last day VCU Health providers will be seeing patients at the neurology clinic located at 501 Lombardy Street, South Hill, VA 23970 will be Sept. 11, 2020.

We will continue to serve our South Hill and surrounding communities with outpatient internal and family medicine and neurology services at the C.A.R.E. building. Neurologist Dr. Yasir Al-Khalili is relocating his practice to our C.A.R.E. Building and will continue seeing patients at this location.

Outpatient family medicine services are also provided at our Tanglewood location (9782 Highway 903, Bracey, VA 23919).

We hope you make the choice to remain with us for your health care. If you wish to continue care with a VCU Health provider in the region, you do not need to transfer your medical records.

  • Those with clinic appointments impacted by the relocation will be contacted to reschedule their appointments.

  • To schedule future family medicine appointments at VCU Health Community Memorial Hospital’s C.A.R.E. Building, please call (434) 584-2273. The VCU Health at Tanglewood location can be reached at (434) 636-6903.

  • To request a copy of your medical records, please contact the VCU Health Information Management Department at (804) 828-4423.

Our decision to consolidate services supports a long-term vision to create ambulatory destination centers. These centers offer the community one convenient access point for a variety of outpatient clinics and complementary services, such as laboratory and imaging services.
