Biography

For Kelli McFarling, M.D., every patient encounter is an opportunity to combine compassion, education and evidence-based care.

A fellowship-trained maternal-fetal medicine (MFM) specialist in the Obstetrics and Gynecology department, Dr. McFarling provides comprehensive care for people experiencing high-risk pregnancies — from managing conditions like hypertension and diabetes to monitoring fetal growth and development.

“Providing high-risk pregnancy care isn’t just about treatment — it’s about educating patients and their families,” she said. “I want to ensure my patients fully understand their conditions and feel empowered by treatment plans that are grounded in the most current evidence. And I'm equally passionate about sharing knowledge with the exceptional medical students, residents and fellows at VCU to shape the next generation of compassionate, evidence-based providers.”

Dr. McFarling knows that risky pregnancies can cause concern and uncertainty — because she’s been there, too. Her daughter was born in the late preterm period “and even though she was only a little bit early, the experience was challenging and gave me firsthand understanding of what it means to navigate an unexpected delivery, the NICU and the emotional weight that comes with it,” she said. “I thought I understood preterm birth, but becoming a patient myself gave me a perspective that training alone could never provide. It made me a more compassionate, empathetic and understanding physician.”

Dr. McFarling earned her medical degree from the VCU School of Medicine and completed her residency in obstetrics and gynecology and fellowship in maternal fetal medicine at the Medical University of South Carolina. At MUSC, she received multiple awards for academic excellence and teaching.

Like many providers on the Women’s Health team, Dr. McFarling also conducts research in her field. She has particular interests in preterm birth prevention. Her studies have led to published papers exploring the role of cerclage (a surgical procedure to prevent the cervix from opening too early during pregnancy) to improve outcomes in periviable pregnancies — those between 22 and 25 weeks where survival is not guaranteed. She also has a strong interest in point-of-care ultrasound, a portable ultrasound technology that allows providers to perform ultrasound exams at the patient’s bedside, enabling rapid diagnosis for maternal conditions.

 “My goal is to ensure that patients feel empowered by their care and that our next generation of physicians carry forward that same patient-centered approach.”
