Sarah Paciulli is a certified family nurse practitioner.  She started VCU Health’s cardiology navigator program in 2015.  The program was born in an effort to enhance the care of cardiology patients and to reduce the hospital’s readmission rate of heart failure (HF) and myocardial infarction (MI) patients.  She runs a transitional care clinic where she sees HF and MI patients in clinic soon after they are discharged from the hospital and follows them in clinic until getting them established with a provider for long-term management. Sarah is also a member of the American Association of Heart Failure Nurses, and is a Certified Heart Failure Nurse.

Prior to her nursing career, Sarah studied Spanish.  She has a Bachelor of Arts in Spanish from the University of Miami.  Paciulli worked as a Spanish medical interpreter for one year before deciding to pursue a degree in nursing.  She is still able to use her Spanish doing outreach with the Spanish-speaking population in the greater Richmond area.  Paciulli volunteers as an NP at Crossover Clinic (one of Richmond’s charitable clinics) where she has provided primary care once a week to Richmond’s underinsured population since 2012.
