Insurance and Billing

  • Please make sure at the time of your appointment that you have the most current copy of your insurance information. Eligibility will be verified prior to your visit. Please notify the staff prior to you appointment if you have an HMO policy as these require prior authorization. Keep in mind that while we are happy to submit all charges to your insurance carrier, you the patient are ultimately responsible for your bill.

  • Co-payments are due at the time of your visit. We accept MasterCard, Visa, Debit cards, checks, money orders, or cash.

  • If you are being seen for a Worker’s Compensation visit, we must have contact with your employer or Worker’s Compensation Representative prior to your appointment. We will need the following information: Claim number, date of injury, employer information (contact name and number), area of body affected, billing information, and authorization for your visit. If this cannot be supplied, your appointment will have to be rescheduled.
