The transplant nurse practitioner is a registered nurse who has additional education to become a certified nurse practitioner. She performs history and physical examinations on the pre and post transplant patients. The transplant nurse practitioner follows patients in the outpatient clinic and helps to coordinate their medical care under the supervision of the attending nephrologist. She monitors patients' conditions and provides other members of the transplant team with appropriate patient information and test results. Nurse practitioners are able to diagnose and treat certain illnesses. They have the ability to write prescriptions for certain medications.
