Medical
VCUHS employees can select either a PPO or a HMO for their medical insurance
needs. Open enrollment takes place in November and plan changes can be
made as a result of significant life events. Highlights of each plan
are as follows:
VCUHS Choice CARE (PPO)
- Administered by Anthem Blue Cross and Blue Shield.
- Primary care physician selection and referrals not required.
- Three physician networks:
- VCUHS network: inpatient care covered in full; office
visit copay $10
- Anthem network: inpatient care subject to copay and coinsurance;
office visit copay $10.00 for primary care, $25 for specialty
care.
- Out-of-network: care subject to deductible and coinsurance.
- Wellness and preventative benefits.
- Pre-natal Management Program.
- Disease Management Program.
- Prescriptions: tiered copays; VCUHS pharmacy discounts;
mail order and retail maintenance pharmacies available.
- Vision: $15 copay for annual exam. Eyeglasses or contact
lenses, laser surgery discounts and mail order replacement
contacts available.
- Out-of-network benefits available.
Anthem HealthKeepers (HMO)
- Administered by Anthem
Blue Cross and Blue Shield.
- Primary Care Physician (PC) manages care; office visit copay
$10.
- Specialist referrals by PCP only; office visit copay
$20.
- Inpatient care copay $250 per admission.
- No out-of-network coverage.
- Prescriptions: tiered copays; VCUHS pharmacy discount;
mail order service available.
- Vision: In-network coverage only; $10 copay for
annual exam. Eyeglasses or contact lenses; laser
surgery discounts;
mail order replacement contacts are available.
- Prenatal Management Program.
- Disease Management Program.
