Coming to a rehabilitation program can be an overwhelming process.  It is important to have a skilled and specialized team working with you to make the process go smoothly and to achieve the best outcome.  At the Rehabilitation and Research Center, we take an interdisciplinary team approach to patient care, in which the patient and family are first and most important part of the team. Along with the patient, the doctors, nurses and multiple therapists work together as one to set goals and monitor progress for the best recovery.  Your team may include:

Physiatrist: (Physical Medicine and Rehabilitation Doctor): is a medical doctor who specializes in rehabilitation. The physiatrist manages the patient's medical issues and coordinates the rehabilitation services while maintaining an open line of communication between patients, families, therapists and other medical services.  Our physiatrists also provides quality follow-up care to their patients once discharged into the community.  Every patient will have access to both an attending physician and a resident physician. 

Nursing Team ( RNs, LPNs, and Care Partners):  provides 24 hour specialized nursing care, focusing of pain management, patient and family education of medications, and the carry-over of the functional techniques learned during therapy sessions.

Physical Therapists (PT): evaluates and provides treatment for patient's mobility needs, focusing on strengthening, endurance, balance and range of motion.  A PT will assist patients with the use of  equipment and modalities to promote the highest level of functional independence.

Occupational Therapists (OT): evaluates and provides treatment for patients with deficits that impact the activities of daily living.  An OT can provide training and adaptive equipment to improve areas of self-care such as dressing, feeding and personal hygiene as well as upper body strengthening to increase independence.

Speech-Language Pathologists (SLP):  provides therapy for patients with swallowing disorders to improve ability to eat and drink.  Patients will also work with an experienced SLP to improve voice and breath support for speech. Patients with SCI, strokes, and brain injuries will work with SLP to focus on improving cognitive-communicative skills.  

Therapeutic Recreation Therapist (TR): provides leisure and recreational opportunities to maximize full potential for patients and provides education for patients and their families on leisure resources and over-coming barriers in the community.

Psychologists and Rehab Psychology (RP):  identifies and addresses psychological, emotional, cognitive and psycho-social barriers to rehabilitation and the successful return to home and community.

Social Workers (SW):  provides support and counseling to the patient and their family to solve issues related to hospitalization.  The social worker will assist in setting up a successful discharge plan and can provide valuable information about services in the community.

Registered Dietitians (RD): evaluates and provides appropriate diet choices for patients as well as educates  patients and family on appropriate and healthy diet selections once discharged home.

Chaplain Services: provides spiritual and emotional care to patients and their families as needed. Weekly Sunday services are provided on the unit in our dayroom.  A chaplain is also on call 24 hours a day for individual services as needed. 

Respiratory Therapy (RT): provides care for patients with acute and chronic breathing difficulties and trach care.

Wound Care Team:  provides care for inpatients with acute and chronic wounds, makes skin care recommendations, evaluates for specialty beds and provides education for wound care and prevention.
