"HOW TO GET INTO REHAB"
A Guide to Inpatient Admissions
Admissions Process:
The Rehabilitation and Research Center strives to make the admissions process here at VCUHS as simple as possible for our patients, their families and referral sources. Referral sources may include physicians, case managers and insurance companies. To ensure maximized benefit from our specialized programs of care, we enable patients to begin the rehabilitation program as soon as they are medically stable. Our team of consult doctors and our admission coordinator meet with patients, families and referral sources daily to evaluate referrals, determine appropriateness and secure medical approval. To speed the process, our admissions coordinator needs to be provided the following information:
- Patient History and Physical ( completed by the physician)
- Physician and Nurses Notes
- Occupational, Speech and Physical Therapy Notes
- Medication Record
- Lab Reports
- Demographic Summary Information
- Discharge Disposition
Insurance and Billing:
The Rehabilitation and Research Center participates with Medicare and Medicaid and most insurance, worker's compensation and managed care plans. We encourage you to speak with your carrier or employer if you have any questions about your policy and coverage. It is also helpful to bring the patient's insurance cards upon admission to ensure the accuracy of the information for billing purposes.
Getting Started:
The Rehabilitation and Research Center hopes to make the admissions process as smooth as possible to get you started quickly toward your goals. Initially, there will be a number of forms to be signed and information to be shared, including consent to begin treatment. Having a loved one on hand may ease the process. It may also be helpful have important cards and documents with the patient and ready to share, including:
- Insurance Cards
- Advanced Directives or Living Will
- Power of Attorney (if applicable)
Please call our admissions coordinator with any questions or fax referrals with the above information to:
