Important Information About Insurance

MAKING AN APPOINTMENT:

If you have decided you would like to schedule an appointment with us, there are two ways you can set one up:

1. YOU CAN HAVE YOUR PRIMARY CARE DOCTOR CALL US:

(804) 323-2255, or FAX (804) 323-2262 or you may call yourself if your insurance company does not require a referral.

Once we have received the request for an appointment we will schedule you for an office visit as soon as possible. Your sleep study itself will be scheduled on a later date after reviewing your medical information and deciding what type of study you need.

2. YOU CAN REQUEST AN APPOINTMENT ONLINE:

If you choose to use this function, please understand that the appointment request will be submitted via email to a Nurse within the VCU Health System and not directly to us. The Nurse that receives your information will then call us with your instructions to contact you for an appointment. Click here to make an online appointment.

The response time is usually two days and they will be unable to answer any specific questions you may have about procedures or insurance. In the future, we hope to streamline online appointments and have the capability to accept these requests ourselves. If you have any questions about your upcoming visit, please call us at 323-2255.

IMPORTANT INFORMATION ABOUT INSURANCE:

The VCU Center for Sleep Medicine participates in most major insurance plans. To find out for sure if your plan covers office visits and sleep testing at our facility you can call the customer service number on your card. All sleep studies are outpatient studies and usually only last about 10 hours. Office visits are only about 45 minutes to 1 hour long.

Some general insurance guidelines for visiting a sleep specialist are below:

HMOs -
Referrals are needed for all office visits with us. If you have an HMO or an insurance plan that requires you to have a Primary Care Physician (PCP), your PCP, regardless of who sets up the appointment for you, must issue a referral number. You may request a referral from your PCP that is good for several visits and diagnostic studies, instead of getting one visit approved at a time.

PPOs -
If you have a PPO plan, a referral is generally not required. However you should first check with your insurance company to see if our sleep specialist (Dr. David Leszczyszyn) is a participant in your network.

OTHER -
For anyone whose insurance does not cover office visits with us, your referring doctor or PCP must obtain authorization for the overnight study.

Your insurer may require a thyroid test (blood work for TSH level) prior to issuing authorizations for overnight sleep studies. If you have had any blood work done recently, please bring a copy to your office visit with us.
